Access the DFSN website to download forms.

Accomplish the necessary forms (see note below).

Submit hard copies of the filled-out forms to the laboratory technician.

Bring UP ID on scheduled laboratory activity.

Please take note of the following:
Keep your intended laboratory activities in the DFSN laboratories within regular working hours (Mondays-Fridays, 8AM to 5PM).
File your permits through the Submission Form at least 5 working days before the intended schedule.
There will be a fee for the use of the laboratory equipment for Undergraduate and Graduate students who will conduct their thesis activities in the laboratories (e.g. Lab 106).
Ensure that you have completed the abovementioned requirements prior to submission.
